We set a goal of making access to your medical records from Allied Eye user friendly. We want to create a better patient-physician partnership and ultimately help us deliver the highest quality of care to you.

To activate your new account, please follow the instructions below:

1. Go to https://iportal.mycareimw.com/alliedeyeps/login/index.php or click the link above.
2. Select "First Time Patient Portal Access" and then click "Continue."
3. Enter your name, date of birth, gender, zip code and the temporary key provided to you during your office visit (or feel free to call us for the key).
4. Follow the easy step-by-step instructions to complete your account set up.
5. Once your information is verified, you can log in and view your account at any time.

After your information is verified, you can log in to view your medical records and more. The portal is fast, convenient, and secure.

While we ensure the patient portal functions as expected, we still kindly ask that you call our office for any medication refills, glasses prescription reprints or scheduling needs.
